Find out if you’re eligible to switch (or switch back) to Health Plan of Nevada Medicaid. You may be eligible if:

  • You live in Urban Clark County AND
  • You changed your Medicaid plan during open enrollment OR
  • Nevada Medicaid changed your Medicaid plan effective on January 1, 2026

Switch by email, mail or phone. Per Nevada Medicaid, switch requests must be received by March 31.

    Send an email to Nevada Medicaid.

    You must have email capability to use this email template. The email will be sent from the email account associated with the mobile device or computer.

    Nevada Medicaid needs the following information to process your change: your name, names of all members in your household you want to switch to Health Plan of Nevada Medicaid, your Medicaid ID number, your address and your phone number. You can also email Nevada Medicaid at nvmmis.mco@gainwelltechnologies.com.
