Apply for Medicaid coverage at DWSS.NV.gov or call 1-800-992-0900, TTY 711. When it’s time to choose a health plan, choose UnitedHealthcare Health Plan of Nevada Medicaid.

If you changed your plan during Open Enrollment, you can change your plan once during the 90-day choice period from January 1 to March 31. Switch to UnitedHealthcare Health Plan of Nevada Medicaid. For more information, call us toll-free at 1-800-962-8074, TTY 711.

Nevada Medicaid needs the following information to process your change:​
