If your Medicaid plan changed on January 1, 2023, you can switch to Health Plan of Nevada’s Medicaid plan.
Request your change now. You have until March 31, 2023 to send in your change form.

Send an EMAIL directly to Nevada Medicaid. You must have email capability to use this email template. The email will be sent from the email account associated with the mobile device or computer.
Nevada Medicaid needs the following information to process your change:
- Your name
- Names of all members in your household you want to switch to HPN’s Medicaid Plan
- Your Medicaid ID number
- Your address
- Your phone number
- You can also send an email to Nevada Medicaid at NevadaManagedCareOpenEnrollment@gainwelltechnologies.com. Please include your name, names of all members in your household you want to switch to HPN’s Medicaid plan, your Medicaid ID number and your address.
